Reablement and Enablement in the City of York

Reablement in York is currently provided by an external company called the Human Support Group (HSG). This service is managed by one of City of Yorks Adult Social Care Teams called The Intensive Support Service (ISS) which is a team of Social Care Workers and Social Workers. 

The Human Support Group (HSG) is a CQC regulated provision managed by the City of York Council and their Intensive Support Service (ISS). It provides short-term, intensive support to reactivate skills, rebuild confidence, and 'reable' people to live independently.

The Intensive Support Service forms part of a system-wide 'home first approach' alongside our external provision, community response, and intermediate care partners.

The procedures in this section of tri.x will support practitioners in the City of York Reablement/Enablement service to effectively assess needs and plan the service then to monitor and review it.
